CANS ASSESSMENT

Comprehensive Adolescent Diagnostic Evaluations with CANS assessments


Psych Wellness Group, Inc. (“PWG”) is now offering Diagnostic Evaluations with

CANS Assessments. CANS transforms the role and process of assessment to play an

integral part in the decision-making, care-planning, monitoring of outcomes and service

responses. PWG would conduct the comprehensive bio-psycho-social intake, complete

and finalize the CANS in the Virtual Gateway (eCAS), establish the initial diagnostic

impression, and create a targeted disposition/referral plan. Depending on the client’s

clinical acuity and needs:


● Youth whose actionable needs indicate standard outpatient therapy can be

smoothly transitioned to clinicians within our group practice who have ongoing

caseload openings.

● Youth requiring intensive community-based wrap-around (such as In-Home

Therapy or ICC/CBHI services) or specialized programming can be formally

linked and referred out to appropriate community agencies, with their finalized

CANS already portable in the Virtual Gateway to expedite their intake.


INTERNAL WORK:

● Portal-Aligned Intake & Real-Time Scoring System: Designed to map

directly to the Massachusetts Virtual Gateway domains during the clinical

interview, eliminating redundant documentation and ensuring immediate

compliance.


● Adolescent Diagnostic Protocols (Ages 12–21): Standardized clinical

trajectories engineered specifically for adolescent mood, anxiety, ADHD, and

school avoidance, moving efficiently through the interview process.

● Standardized Conclusion & Transition Protocol: A formal clinical discharge

document certifying the state submission and transferring the family directly to

their ACO/CBHC care coordinator, cleanly terminating our evaluation scope

when an internal therapist is unavailable.

● Collateral & Consent Infrastructure: Dedicated consent packets for

pediatricians and high school / middle school guidance counselors to ensure all

outside records and communications are legally protected before outreach

occurs.


CRITERIA TO QUALIFY:

PWG is not accepting cases for ongoing DCF/51A coordination, forensic evaluations,

court-ordered evaluations, or crisis management.


Referring providers must confirm these criteria before an intake is scheduled:


● Target Demographic: Scope is strictly focused on adolescents (middle and

high school ages); we are not accepting early childhood or latency-age

referrals for this service line.

● True Outpatient Fit: The teen is seeking routine outpatient therapy with PWG,

not intensive emergency or daily in-home wrap-around care (IHT/CBHC).

● Clinical & Psychiatric Stability: No acute psychiatric hospitalizations, partial

program step-downs, or emergency department visits within the past 6 months.

● Absence of Active Child Welfare/Crisis Involvements: No open, contested

DCF investigations, active 51A filings, or court-ordered evaluations, which

require specialized forensic or state agency management rather than

outpatient diagnostic slots.

● Legal Readiness: Full caregiver alignment, with all legal custody agreements

or divorce decrees uploaded to the portal prior to booking.


Final determination of appropriateness for outpatient telehealth services will be made during the

intake assessment. If significant or urgent safety concerns are already known at the time of

referral, including active suicidal intent/plan, escalating self-harm, acute psychosis/mania, or

significant aggression, a higher level of care or crisis evaluation will be recommended rather

than a routine outpatient intake. PWG is not a crisis service and does not provide

emergency psychiatric evaluation, crisis stabilization, PHP, IOP, inpatient care, or other

intensive services
